Please follow the final steps to complete your Mass Mutual disability insurance policy!

1. You will be receiving an email from Mass Mutual’s system. You will need to eSign the delivery forms so we are able to finalize your policy. The EFT form is marked to draft the payment at delivery as well as recurring monthly payments. Monthly payments require automatic withdrawal. Please make sure to select your checking or savings account. If you would like to pay in installments other than monthly, please contact us!

2. Download a copy of your policy from the DocuSign link sent to you.
